That’s why we developed SureStep Fall Prevention: a HomeWell Special Program that protects older adults from falls and the risk of falling.
SureStep starts with an assessment by a HomeWell Care Manager: an expert on fall risks and the factors that contribute to them. They’ll design a custom care plan to address your fall risks holistically, so that you or a loved one can protect your independence at home.
With a custom in-home care plan designed by an expert Care Manager, SureStep addresses the following factors through activity, engagement and more.
Physical Inactivity
Sitting for long periods can result in muscle weakness and loss of balance. Our caregivers can help your loved one do more physical activity in safety.
Medications
Medications and their side-effects can increase fall risk, especially with multiple prescriptions. We assess whether your loved one’s medication increases their risk of falling.
Hazards in the Home
We evaluate your loved one’s home for safety issues like poor lighting and trip hazards, and can recommend modifications for better safety and access.
Low Vision
Having impaired vision doubles an older adult’s risk of a fall (2). We work with clients to understand and address the unique fall risks that come with low vision.
Prior Fall History
An older adult’s first fall doubles their chances of falling again(3). It can also cause a fear of future falls, which may cause decreased activity and a sense of withdrawal.
Social Isolation
Inactivity and social isolation lead to loneliness, depression and anxiety, which increase the risk of falling. Our caregivers can help your loved one do more of the activities they enjoy, with a companion.
Multiple Chronic Conditions
Chronic conditions often reduce strength, balance, and mobility. Our caregivers are trained to work with clients with multiple conditions that present a heightened fall risk.
